ABSTRACT

@#Punctate inner choroidopathy(PIC)is an infrequent idiopathic chorioretinopathy, frequently affecting young myopic women. There are multiple, small, round, yellow-white or gray punctate lesions in the retinal pigment epithelium and the choroidal lining, in the absence of anterior ocular segment or the vitreous body inflammation. In general, most patients have good prognosis and a few patients will be caused severe vision loss if complicated by choroidal neovascularization(CNV)and subretinal fibrosis. This article reviews the etiology, pathogensis, clinical manifestations, diagnosis and differential diagnosis and treatment of PIC.

ABSTRACT

Objective To compare the prenatal diagnostic value and image quality of magnetic resonance imaging(MRI)with fast imaging employing steady state acquisition(FIESTA)or single shot fast spin echo(SSFSE)sequence,in order to provide references for sequence selection of prenatal diagnosis. Methods The MRI data of 121 patients with suspected placental invasion were retrospectively analyzed. The ability of FIESTA in displaying MRI signs associated with placental invasion and its image quality were assessed and compared with SSFSE. Based on the records of cesarean section and pathological finding, the sensitivity, specificity, positive predictive values, and negative predictive values of these two sequences were calculated. Results The image quality was significantly higher in FIESTA than in SSFSE(χ =29.74,P=0.000). FIESTA had significantly higher ability to display focal interruptions in the myometrial wall than SSFSE(χ =6.750,P=0.006);in addition, the ability to display abnormal vessel in the placenta(χ =8.471,P=0.020),placental heterogeneity(χ =13.885,P=0.000),hypointense intraplacental bands(χ =4.267,P=0.035)were also significantly higher in SSFSE than in FIESTA,while the efficiency for displaying uterine bulging(χ =0.250,P=0.625),uterine recess(χ =0.167,P=0.687),uterine penetration and parametrium implantation(χ =0.800,P=0.375),and protrusion of the placenta into the cervix(χ =0.081,P=0.776)were not significantly different between these two sequences. Both sequences had a specificity of 100% in displaying uterine penetration and parametrium implantation, uterine recess, and protrusion of the placenta into the cervix. Conclusions FIESTA has better ability in displaying the contour and demarcation of placenta and uterine, whereas SSFSE is more efficient in displaying the changes of intraplacental signals. FIESTA can be used to observe the relationship between the placenta and the surrounding structures and whether the surrounding tissue is implanted, and the changes of placental signals can be observed in SSFSE. The combination of these two sequences can improve the prenatal diagnosis of placenta invasion.

ABSTRACT

AIM:To evaluate the long-term clinical efficacy of Shotfile software for the myopia patients with presbyopia.METHODS:A total of 43 cases (82 eyes) over the age of 35 myopia patients were treated with laser corneal surgery in our hospital, added Shotfile software at the same time, and followed up for 3a.The uncorrected visual acuity(the distance and near vision),diopter change and visual satisfaction rate were observed at 1,2 and 3a after operation.RESULTS:At 1a after operation, there were 81 eyes (99%) with the uncorrected distance visual acuity reached the preoperative best corrected visual acuity, and 75 eyes (91%) with the uncorrected near visual acuity above 0.66,the average value of the spherical equivalent was (-0.64±0.63)D,the overall satisfaction rate was 91%.At 2a after operation, there were 79 eyes (96%) with the uncorrected distance visual acuity reached the preoperative best corrected visual acuity, and 69 eyes (84%) with the uncorrected near visual acuity above 0.66,the average value of the spherical equivalent was (-0.62±0.59) D,the overall satisfaction rate was 86%.At 3a after operation,There were 77 eyes (94%) with the uncorrected distance visual acuity reached the preoperative best corrected visual acuity, and 62 eyes (76%) with the uncorrected near visual acuity above 0.66,the average value of the spherical equivalent was (-0.55 ± 0.56) D,the overall satisfaction rate was 81%.Intraoperative or postoperative serious complications were not found in all the cases,1 eye of spherical equivalent with-10.0D was found-1.75D back after 1a,respected the willingness of patient for the second correction.CONCLUSION:Shotfile software for the patients myopia with presbyopia is safe and stable in the long-term clinical treatment, and the patient satisfaction is higher, but the near visual ability of some patients may be weakened with the prolongation of time, preoperative communication with patients should be fully.

ABSTRACT

Scientific research output of clinical trials in Chinese medicine (CM) is insufficient, but international papers hold an important scientific position in China. Based on the current situation, we analyzed the present publication situation of international medical papers in our country, and the feasibility and urgency of publishing international papers on clinical trials of CM. Finally, we proposed to use the PDCA (plan-do-check-action) cycle method to improve the quality control and management of CM clinical trials. Moreover, by combining our experience in relevant scientific research launched at our department, we expounded strategies for improving the quantity and quality of international papers in CM.
